Membership in the Květinový klub
Every physical entity older than 16 years with correspondence address at the Czech Republic, who agrees with the general conditions, can become a member of the Květinový klub. Body corporate cannot become a member of the Květinový klub. Membership is complimentary. Every customers of the Květinový klub are entitled to become members, after fulfilling club application and agree with the general conditions of the discount programme. Members activate their club cards on the web page www.kvetinovyklub.cz, where they fill in every requested detail. Activation of the club card is done by Květinový klub when registering at one of our shops.
Application form
Enabling of personal information through this application form is voluntary, however without filled up mandatory fields members cannot utilize the Květinový club benefits. In accordance with the Czech code of law #101/200 Sb., about the protection of the personal information and its further changes, company Květinový klub, Sixth Sense s.r.o. with the seat Na Balkáně 2120/134, 130 00 Praha 3 commits, that all information in this application form will not be disclosed to any third party without member´s permission.
